Seamless Transition

What is it?

Transition teams work to support individual students moving toward their postsecondary plans for employment, education, and independent living. The goal is to have plans for employment, education, and community living in place before leaving high school. The intentional planning and collaboration for that success is called a seamless transition.

Why is it important?

Indiana’s Department of Education and Division of Disability and Rehabilitative Services (DDRS) hope to increase the number of individuals with disabilities in competitive integrated employment from 23% to 38% by 2027. Therefore, they invested in a pilot program at eight sites to guide schools, interagency transition teams, and families in implementing seamless transition tools and processes. CCLC and INSTRC researchers gained valuable insights through focus groups and interviews, which they will use to guide other districts interested in joining this movement.
